Heart Failure is a weakening of the heart's pumping power. With heart failure, your body doesn't get enough oxygen and nutrients to meet its needs.

The Appropriate Care Measure (ACM) is a patient-centered composite score that captures whether or not a patient received all of the recommended treatments they were eligible to receive, based on the specific condition.

A quality measure is medical information from patient records coverted into a rate or percentage that shows how well hospitals care for their patients. You can use this quality information to help you compare hospitals.
