Pre-Registration
It is important to pre-register for you hospital stay by completing a Maternity Pre-Admission Form. You should complete this form at least four months before your due date. You may pre-register in three different ways:
2. Contact your obstetrician's office and request Presbtyerian's maternity pre-registration form.
3. Request the maternity pre-registration form by contacting Presbyterian at 704-384-4949.
The information on your Maternity Pre-Admission Form will be used to admit you to Presbyterian Hospital, Presbyterian Hospital Matthews or Presbyterian Hospital Huntersville. Please allow approximately 21 days for your information to be processed.
Processing
You will receive a verification letter within 3-4 weeks after you mail or e-mail us your form confirming that we have received your information. We will begin verifying your insurance information and preparing for your admission.
