The medical record is the property of the hospital and it must be completed by all physicians involved in the patient's care before it can be copied and released to the patient. Therefore, the patient may not receive a copy of his/her medical record upon discharge from the hospital.
Patient related information may be released to a physician or medical facility for follow-up and continued care of the patient when needed.
Your medical record is a confidential document, so your written authorization is required before a copy can be released. If you're 18 or older, you can get an authorization form from the Medical Records office. Call 704-384-4150. A fee may be required to provide your records.
