Heart failure is a serious form of heart disease, but it can be monitored and treated in an attempt to improve heart function and reduce symptoms.

The Presbyterian Heart Failure Program offers an outpatient, intensive clinic that provides comprehensive care to people with advanced heart failure. This program aims to help patients improve their quality of life and decrease hospitalizations. A self-management program is also available. Both programs are offered in Charlotte, Huntersville and Matthews and are led by a team of heart failure specialists.

The team includes:

  • nurse practitioner
  • nurse case manager
  • dietitian
  • social worker (if needed)
  • resource specialist (if needed)

Heart Failure Clinic

Our team develops individualized plans for patients with advanced heart failure. Patients receive a detailed evaluation and are monitored according to their individual needs.

This program includes regular visits to one of the Presbyterian Heart Failure Program locations. During each visit, patients are examined by a nurse practitioner and are assessed for daily weight, diet, exercise level, medications, fluid intake and smoking cessation (if necessary).

Participation in the Heart Failure Clinic requires individual consent and a referral from a cardiologist. Patients are encouraged to continue seeing their primary care physician and cardiologist while they are enrolled in the clinic. Our team maintains ongoing communication with those physicians.

Heart Failure Basics 101

Heart Failure Basics 101 promotes self-management by enabling patients to learn more about their condition and detect early changes in their health. Benefits include:

  • in-depth education
  • emphasis on daily weight and symptom monitoring
  • strong focus on early intervention for heart failure triggers
  • connection to community resources
  • access to heart failure navigator
  • educational materials and tools

Heart Failure Basics 101 consists of a one-time office visit with our team of heart failure specialists. After the office visit, telephone follow-up is provided for three weeks.
