Adult Programs - On admission, staff members will assign an identification number to all patients. To protect your confidentiality, we do not release information regarding your admission, room number, or telephone numbers, even to your relatives, without a correct identification number. You or family members are responsible for giving the number to persons you wish to know you are hospitalized here. Persons who call or attempt to visit and cannot provide the correct number will be told that we are unable to establish if you are a patient here.
Child and Adolescent Programs - Information about the patient will be shared only with the team and others for whom a current and valid release of information is signed. You have the responsibility not to divulge any information regarding other patients or families with anyone outside the Child/Adolescent and Family Program. The patient is assigned an ID number to protect his/her confidentiality. Only parents and legal guardians will know this ID number. Treatment information concerning the patient will not be given to anyone who does not have the ID number. Staff prefers to give information only to parents and legal guardians. ID numbers are also necessary for entry onto the unit.
