Living With Diabetes is an extensive self-management program that helps people with diabetes live active, independent and vital lives.
The Living With Diabetes self-management program consists of three parts:
You will learn about self-management, the skills needed to control blood sugars, and information on diet, monitoring, meal planning and exercise.
You will meet with the nurse and dietitian to monitor your progress, to continue your self-management training, and answer your questions.
You will learn how to address common issues such as the highs and lows of blood sugars, management on the days that you are not feeling well, stress management, carbohydrate counting, eating out, and chronic complications.
A physician referral and preassessment are required for eligibility for this class. We will create a customized program tailored to meet your needs. To learn more, please call 704-384-7390.
