Completing our Pre-Admissions Form long before your baby is due will help us prepare for your special delivery.

Pre-Registration
It is important that you pre-register to deliver your baby by completing a Maternity Pre-Admission Form. It is best to complete this form at least four months before your due date. This form can be obtained from your obstetrician's office or by calling 704-384-4949. The information on your Maternity Pre-Admission Form will be used to admit you to Presbyterian Hospital or Presbyterian Hospital Matthews. Please allow approximately 21 days for your information to be processed.
Processing
You will receive a verification letter within 3-4 weeks after you mail or e-mail us your form confirming that we have received your information. We will begin verifying your insurance information and preparing for your admission. If you have any questions regarding the status of your Pre-Admissions Form, you may contact our Pre-Admit staff at 704-384-4879.
At Presbyterian, even the admissions process is designed to make life easier for you. Click here to complete our Pre-Admissions Form on-line. You will receive confirmation that we have received your information with 3-4 weeks of completing the form on-line. Adding your email address to the online form will increase the confirmation response time.
