Hospice Care Team
Learn more about the professionals who make up our hospice care team.
The hospice care team meets weekly to discuss each patient's case and ensure that his or her needs are being met. Our team of hospice professionals may include:
   
Physician Supports the hospice team on medical issues and suggests treatment
Registered nurse Monitors the symptoms of the patient's disease and discusses possible treatment options
Medical social worker Provides family counseling and assists with long-term planning and resources to meet the patient's needs
Certified nursing assistant Assists patients with personal care, home exercises and activities of daily living
Physical therapist Assists and instructs family/caregivers on special safety needs
Occupational therapist Assesses the patient's home and suggests modifications to make daily activities easier
Speech therapist Assesses speech, swallowing, and non-oral communication and language difficulties
Chaplain Provides spiritual comfort for patients and their families and supports the patient's individual faith organizations
Volunteers Provide companionship and respite care for patients and their families, assist with transportation needs and errands, and offer bereavement care to the family
