Heart Failure Treatment
The Presbyterian Heart Failure Clinic offers disease management programs that are designed to help patients understand and live with heart failure.
Heart failure is a serious form of heart disease, but it can be monitored and treated in an attempt to improve heart function and reduce symptoms.

The Presbyterian Heart Failure Clinic offers an at-home self-management program as well as an outpatient, intensive clinic that provides comprehensive care to people with advanced heart failure. Both programs aim to help patients improve their quality of life and decrease hospitalizations.

The Telehealth Program is specifically designed for patients who have mild to moderate forms of heart failure or who are in the early stages of the condition. Patients in this program receive expert guidance and tools to help them develop self-management skills at home.
Presbyterian Heart Failure Program
Our team of skilled nurse practitioners, nurse case managers, social workers, dietitians and occupational therapists develop individualized plans for patients with advanced heart failure. Patients receive a detailed evaluation and are monitored according to their individual needs.

This program includes regular visits to the Presbyterian Heart Failure Clinic. During each visit, patients receive a physical exam and are assessed for daily weight, diet, exercise level, medications, fluid intake and smoking cessation (if necessary).

Patients are encouraged to continue seeing their primary care physician and cardiologist while in the Heart Failure Program. Our team maintains ongoing communication with referring physicians.

A monthly support group is available for patients who are interested in connecting with other individuals facing heart failure.
Participation in the Presbyterian Heart Failure Program requires individual consent and a referral from a cardiologist. Referring physicians receive regular updates from our team.
