Heart Failure Telehealth Program
| The Presbyterian Heart Failure Telehealth Program is an educational disease management program for patients who have mild to moderate forms of heart failure or who are in the early stages of the condition. |

The Telehealth Program is designed to help patients take control of their condition. Our goal is to give patients the tools they need to develop self-management skills in the comfort of their own homes.
The program features a team of heart failure specialists that includes a:
- Nurse practitioner
- Nurse case manager
- Dietitian
- Social worker
- Resource specialist
This team does not take the place of patients' regular physicians; instead, we work with physicians to monitor patient conditions and develop and maintain the best possible care plan.
Patients begin with a one-time visit to the Heart Failure Clinic for an assessment and education and to receive take-home materials, including a special scale. Patients use the scale at home to check their weight each morning and to answer a series of questions. The data collected by the scale is sent back to the Telehealth team. The team checks in with patients at regular intervals throughout the program.

The Telehealth Program is designed to last for approximately three months. At that time, one of three things may happen:
- If the patient has developed the skills necessary to maintain a healthy lifestyle, he/she will graduate from the program.
- If the patient is not quite ready to leave the program, enrollment is extended for one to two months.
- If the patient's condition has not improved, he/she will be transitioned into the Presbyterian Heart Failure Program, which is designed for patients who require more vigorous support and onsite visits.
