When you are admitted to Novant Health Charlotte Orthopedic Hospital, our team will ask questions to identify whether you need additional assistance after you are discharged. We will work with you, your family, and your physician to plan for your discharge during the entire time you are in the hospital.
Your physician, nurse, case manager, bedside nurse, and physical therapist will determine when you are ready be discharged from the hospital. Your physician will provide a written authorization of discharge. At that time, your physician and/or discharge nurse will review any instructions for follow-up care with you. You will receive a copy of these instructions. Discharge time is approximately 11 a.m. Please plan to have someone available to pick you up at this time. Your discharge plan may include information about:
- Medications
- Special diet and foods to avoid
- Use of equipment
- Available community services
- Care of your incision
- Activities of daily living
- Safety measures
- Follow-up appointments and care
- Activity
- Pain management
- Financial concerns
- Lifestyle changes, such as stopping tobacco use and weight loss.

If you have concerns related to your care or recovery after you are discharged, contact your physician. In your discharge instructions, we will tell you how to contact your physician and other information.
