Heart failure is a serious form of heart disease, but it can be monitored and treated.

The Novant Health Heart Failure Program is an intensive outpatient program that provides comprehensive care to people with advanced heart failure. Our program is designed to help improve quality of life and decrease time spent in the hospital. Programs are offered in Charlotte, Huntersville and Matthews and are led by a team of heart failure specialists.

The team includes:
  •  Nurse practitioner
  •  Nurse case manager
  •  Dietitian
  •  Social worker (if needed)
The Novant Health Heart Failure Clinic is an important part of our program to help those with advanced heart disease. Working with your cardiologist and primary care physician, our team develops an individualized program including a detailed evaluation and monitoring plan to address your specific condition.

The Heart Failure Clinic is the central point for coordination of post-hospital care. If you have heart failure, you may be asked to visit the clinic. During each visit, a nurse practitioner will check your weight, diet, exercise level, medications, fluid intake and tobacco use. The clinic staff works with you to help you understand self-care issues such as healthy eating, exercise and managing stress. The staff will also communicate with your family physician or cardiologist to make sure you get the right follow-up care after you leave.

What we have discovered is that those patients who come through the Heart Failure Clinic have a lower readmission rate to the hospital than those patients who do not use the clinic.

To participate in the Heart Failure Clinic you will need a referral from a cardiologist.

Heart Failure Basics 101


Heart Failure Basics 101 is designed to help you understand your condition and how you can take an active role in managing it. When you participate, you receive:
  •  A full assessment by a nurse practitioner
  •  Education on your medical condition and how to manage it for a better quality of life
  •  Help finding community resources
  •  The support of a heart failure navigator who guides you through the treatment process.
Heart Failure Basics 101 is a one-time office visit with our heart failure team. After the office visit, the team will follow up with you by phone to answer questions and help with any problems you may be having. Call 704-384-8888 to learn more.
